Working under the supervision of the Diabetes Program Manager, the Diabetes Technology Patient Navigator enables the coordination of multidisciplinary follow up care for patients and the coordination of diabetes technology devices for insulin pumps and Continuous Glucose Monitors (CGM). This individual will also serve as the liaison for specialty services and will work closely with our other Diabetes Program locations and clinicians to create an efficient and patient-centered system for scheduling multidisciplinary new diabetes clinic appointments as well as providing the necessary loop closure for referring physicians.
- Receiving requests from Providers via Electronic Medical Record (EMR) for specific diabetes technology prescriptions for their patients.
- Overseeing Diabetes e-fax server, Faxcom.
- Communicating with clinical staff teams within the Endocrine Division (i.e.: Pharmacy technicians, Access Center, etc.)
- Gathering and submitting necessary patient data to fulfill PA requirements, specific to insurances (i.e.: Laboratory data, clinical notes, and patient-collected data obtained on web-based secured portals).
- Creating and maintaining relationships and open lines of communication with representatives at DME companies, for rapid processing of prescriptions.
- Connecting and communicating with Providers and patients to support ongoing access to prescribed devices or supplies.
- Connecting with patients when devices are authorized and instructing them on how to obtain their devices.
- Cross-trained with the Diabetes Patient Navigator for Special Programs, to provide additional coverage during planned vacations.

- High School Diploma required, bachelor’s degree preferred.
- A minimum of 2-3 years’ experience in a healthcare environment required with experience in patient navigation, public health, or patient care coordination in a complex medical office preferred
- Bi-lingual strongly preferred, with Spanish or Haitian-Creole as second language
- Ease in interacting with patients, some of whom have multiple comorbidities, including social challenges.
- Exceptional judgment and discretion in interacting with physicians and staff on sensitive political and confidential issues.
- Knowledge of current issues in the healthcare environment preferred.
- Familiarity with information systems preferred.
- Experience in working with Windows 10 software (i.e.: Word, Excel, Powerpoint, etc.) required.
- Experience with working with a patient-oriented database, such as RedCap, preferred.
