**Responsibilities**:

- Know and understand the vision, values, and goals of Desire Home Care.
- Monitor department performance metrics and ensure Care Coordination team accountability to meet or exceed these targets.
- Demonstrate proficiency in learning and implementing agency policies and procedures to ensure compliance and effective monitoring.
- Oversee and train team to support departmental and organizational objectives.
- Achieve 100% utilization of Consumer Authorization to ensure full resource allocation.
- Achieve 100% utilization of Lead and Routine caregivers' ensuring caregivers are fully utilized based on their availability.
- Guarantee high-quality care services for both consumers and caregivers.
- Address and resolve any complaints or concerns from consumers and caregivers swiftly.
- Verify and ensure team has obtained all essential documents and uploaded them into the care system before starting services.
- Verify that consumer assessments and other data have been accurately and promptly entered into the system.
- Develop and maintain trusting relationships with the team, consumers, and caregivers.
- Cultivate and sustain partnerships with agency referral sources.
- Conduct performance evaluations for supervised staff.
- Manage, review, and implement disciplinary actions and forms.
- Mentor departmental staff and assist in the development of their personal development plans (PDPs).
- Undertake additional duties as requested by supervisors or management.
- Enhance Desire Home Care’s learning-oriented work culture by actively contributing to a positive environment.
- Serve as the primary point of contact for clients, their families, and caregivers, ensuring their ongoing satisfaction with our services.
- Respond promptly and effectively to all client & caregiver questions, concerns, and complaints.
- Collaborate with agency referral sources as needed.
- Convey outbound messages with a focus on professionalism, emphasizing thoughtful word choice, grammatical accuracy, and a tone that reflects emotional intelligence.
- Proactively identify and address potential issues before they arise per company policies.

**Requirements**:

- Minimum of two years of experience in Home Healthcare, Consumer or Patient Care Management, or a related field
- Minimum of 4 years of experience managing a department/team
- Bachelor’s degree or equivalent educational background preferred
- Bilingual in Spanish
- Strong customer service orientation with excellent negotiation and critical thinking skills
- Proven ability to work independently and collaboratively in a dynamic environment
- Proficiency in MS Office, particularly Excel and Outlook, and experience with standard office equipment
- Capable of interpreting key performance indicators (KPIs)
- Excellent communication and interpersonal skills
- Ability to prioritize effectively in a fast-paced environment
- Valid driver's license and reliable transportation
- Business casual attire
- Negative TB test within 30 days of hire
- CPR & First Aid certificate within 30 days of hire

**Work Schedule**:

- Monday-Friday, 8:30A-5:30P
- Rotating on-call schedule
- Hybrid after 90-day probation period
