**Minimum Qualifications**
- Valid New Jersey RN license

**Job Duties**
- Assess and appraise the quality of care for long-term residents.
- Conduct comprehensive interviews and evaluations of incoming residents.
- Develop structured care plans for short-term and long-term goals.
- Coordinate with multidisciplinary treatment teams, caregivers, and families.
- Facilitate in-service education sessions on MDS, RAPS, and Triggers.
