**GULF COAST CENTER - JOB DESCRIPTION**

**Job Title**:Coordinated Specialty Care QMHP/SES (Supported Education Specialist)

**Location**:Serves Brazoria and Galveston County, Texas. Must be available to work in-person

**Schedule**: 8am-5pm, and on call rotation for crisis calls for individuals enrolled in CSC

**FLSA Status**:Non-Exempt

**Working Status**:Full Time with Benefits

**Summary**:
This position provides early intervention services for individuals with First Episode Psychosis between the ages of 15 to 30 by providing skills training in the areas of Supported Employment, Supported Education, and Case Management to enrolled individuals with chronic mental illness in the community or a home setting. Provides services as a Qualified Mental Health Professional (QMHP) utilizing evidenced-based curriculum as required by FEP Program. This position performs services according to the individual's Recovery Plan, Gulf Coast Center, and Medicaid standards and requirements. Provides client services at the service site and in vivo services within a natural community setting. This position's Key tasks include
- Completing ANSA and standardized assessments to determine need for FEP services,
- Developing service goals/objectives in collaboration with the client
- Participating in the development of the client's Recovery Plan in collaboration with the client and other team members
- Providing individual skills training according to authorized curricula,
- Assessing client's progress in meeting service goals/objectives,
- Insuring client/family satisfaction with services, participating in updating Recovery Plan as needed,
- Coordinating the completion of responsible staff activities,
- Completing required documentation accurately and in a timely fashion.
- Established competency in IPS Supported Employment, Supported Housing, Crisis Related services and COPSD.

**Job Duties: (To be performed with or without accommodation as based on ADA requirements)**

**The Coordinated Specialty Care QMHP will be responsible for the following essential Job Duties**:

- Identifies a current and discreet caseload as part of an interdisciplinary team.
- Serve as a member of the treatment team coordinating and providing services as needed.
- Actively participate in team meetings as evidenced by discussion that is not only specific to assigned caseload.
- Display professionalism and team membership when representing program.
- Personnel vacancy may result in temporary increase of caseload.
- Maintain contact with designated staff on a daily basis regarding current work area and retrieval of messages.
- Provides RCM self-management strategies and clinical coordination to individuals, groups, and family members as defined by GCC using evidence-based strategies within the (TRR) model of care.
- Provides crisis services to consumers and walk-ins to the clinic as needed.
- Provides crisis services for after-hours response including nights, weekends, and holidays to CSC clients per rotation among qualified staff on the CSC team.
- Established competency in IPS Supported Employment
- Provides monthly documented face-to-face hours of direct service per month to GCC consumers and/or family members according to the current Direct Service Agreement.
- Assure treatment provided to individual relates to the individualized goals of Person Centered Recovery Plan.
- Utilizes a variety of skills training in conjunction with the IMR (Illness Management and Recovery Model) and may utilize the following intervention strategies: instruction, positive feedback, repetition, role play, behavioral reversal, in-vivo practice and coaching.
- Enters into EHR, accurate and appropriate documentation within 2 business days of services per TAC and GCC documentation policy requirements
- Maintains compliance with all documentation for consumers as assigned and as required by the GCC policies and procedures within caseload.
