**Per Diem Case Manager**:

- Weekly Hours: 20-40
- $31.50/hour for billable hours

**Description**:
Licensed by the Department of Health and Human Services, Gateway Community Services Case Management program serves eligible youth and adults. Through a variety of services from assessment through advocacy and evaluation, we seek to encourage and build independence, enhance functioning in home, school and community, and empower children and adults to achieve their optimal potential.

Case managers will first conduct an initial assessment to determine the individual’s medical necessity and eligibility. The assessment reviews the medical, mental health, educational, social and recreational needs. Next, we will develop an individualized treatment plan focusing specifically on the goals and needs identified by the child or adult we are serving. With a better understanding of the individual’s strengths and needs, the case manager can then help find services and support, make referrals, assist with team meetings and education needs, and generally monitor progress to ensure the effectiveness of the plan in meeting goals. Right from the start, the individual or family is involved and up-to-date in the progress made every step of the way.

Our Case Managers can:

- Connect other with mental health and medical services in your community
- Connect and advocate for education services
- Connect to housing and financial resources
- Assist with navigating immigration needs
- Assist with scheduling and coordinating appointments
- Secure transportation needs through Modivcare for routine appointments
- Support with finding recreational or social activities
- Collaborate with providers to develop crisis and wellness plans
- Support in managing challenging mental health symptoms

**Required Certificates and Licenses**
- Valid driver's license and in good standing
- Bachelor Degree in Social Work or related field
- MHRT-C _(Have either attained and maintained MHRT-C or be able to attain a _**provisional **_MHRT-C which has a 90-day probationary period) _
