**ORGANIZATION OVERVIEW**:
The mission of the Community Health Care Association of New York State (CHCANYS) is to champion community-centered primary care in New York State through leadership, advocacy, and support of Community Health Centers (CHC). To do this, CHCANYS serves as the voice of community health centers, working to ensure that every New York State community has primary care that encompasses all aspects of each patient’s health and well-being. CHCANYS is a dynamic, team-oriented, 501(c)(3) non-profit organization, and as New York State’s Primary Care Association (PCA), advocates on behalf of and provides technical assistance to federally qualified health centers (FQHCs) throughout New York State. New York’s nearly 800 FQHCs sites provide comprehensive, community-based primary and preventative care services to 2.2 million New Yorkers annually, regardless of insurance status or ability to pay. CHCANYS is nationally recognized for its programs in health policy, performance improvement (PI), health information technology, health system planning, and training and technical assistance (T/TA).

**DEPARTMENT OVERVIEW**:
CHCANYS’ Clinical Excellence and Innovation (CEI) Department serves New York State’s Community Health Centers, also known as Federally Qualified Health Centers (FQHCs), as a resource hub of primary care, health equity and practice transformation, performance improvement, and clinical informatics training and technical assistance. The CEI Department facilitates FQHC’s success in value-based healthcare and payment arrangements by promoting health equity across vulnerable populations and disseminating evidence-based practices to maximize efficiency and performance in metrics related to health disparities, prevention, chronic disease management, and appropriate utilization; driving operational excellence by optimizing the use of clinical informatics; building competency and capacity in population health management, care management and coordination, team-based care, and patient engagement; and promoting integration with the Medical Neighborhood by building community linkages and partnerships and preparing CHCs to practice according to advanced primary care modules.

The Project Coordinator plays an integral role for the CEI team, helping to manage a diverse portfolio of projects that span health equity, chronic disease prevention and management, and overall practice transformation and value based care. The Project Coordinator supports the team by providing logístical support in their project management needs, including data collection, analysis and presentation, development and resource dissemination necessary for both internal and external stakeholder meetings, including collaborative peer-learning and sharing opportunities for CHC staff. The Project Coordinator also assists in identifying and implementing solutions to facilitate a more integrated, cohesive and streamlined team through the judicious use of strong facilitation and communication skills, and program management tools. This position will also support CHCANYS in its unique and essential role, along with its member FQHCs, in addressing health disparities and our DEI efforts and policies across CHCANYS. This position is partially grant-funded.

**ESSENTIAL DUTIES AND RESPONSIBILITIES include the following**:
**Responsibilities**:

- Serve as the central hub and key point of coordination for CEI staff and projects, coordinating daily functions of the CEI team to enhance T/TA program services, including:

- Participating in T/TA content development and delivery design
- Managing reporting analysis and visualization of performance data from the Center for Primary Care Informatics (CPCI) statewide data warehouse to contribute to planning and delivery of T/TA services
- Coordinating and supporting T/TA webinars, conference calls, in-person trainings, external presentations, and practice facilitation site visits
- Managing and maintaining CRM-related data on FQHCs, including participation in various initiatives (e.g., PCMH, MU, DSRIP), CHCANYS projects and T/TA activities, key health center staff contact information, and listservs
- Strengthen the function of department efficiency by identifying new solutions (i.e. technology, processes) that will improve communication, collaboration and satisfaction
- Coordinating and scheduling weekly CEI huddles and team meetings, preparing meeting materials and minutes, and disseminating information among CEI staff
- Collaborate with CEI team and health center staff to coordinate content and workshops for the annual CHCANYS conference
- Perform other duties as assigned to fulfill CHCANYS’ mission
