**JOB FUNCTIONS**:
**PROGRAM/DEPARTMENT RESPONSIBILITIES**:

- Utilize marketing and promotional materials and resources to increase awareness of Evidence Based Heath Interventions and Health Awareness Months.
- Identify innovative ways to conduct outreach to connect participants and referring providers to the programs;
- Identify and address health disparities to ensure health equity;
- Actively engage members and recruit for EBHI programming;
- Manage program leads and waitlists for each program;
- Conduct the day to day functions of EBHI/ Healthy Living programs:

- Ensure data is entered accurately and timely;
- Enroll participants in program classes and collect fee where necessary;
- Ensure programs are being delivered with fidelity to evidence base including working with other program instructors within the branch.
- Promote programs to healthcare providers in branch community to increase program referrals with a focus on reaching under-served populations;
- Regularly provide updates and feedback to referring providers;
- Communicate and report regularly (weekly) with Healthy Living Program Director as to the status of each EBHI or Program;
- Work with branch and Healthy Living Program Director to identify and hire additional program instructors as needed depending on program growth and budget;
- Regularly participate in Association wide EBHI staff team calls/ meetings(monthly and as scheduled);
- Work closely with Healthy Living Program Director to align branch EBHI's with the Association offerings;
- Work closely with branch Membership and Health and Wellness team to operationalize EBHI;
- Ensure program participants are connected to the Y through membership and to other relevant programs for them and their family.

- Adhere to all fiscal policies and procedures such as petty cash, member services, member registration, purchase orders, and employee expenses.

- Participate as requested in branch committees, coalition and task forces
- Represent the OCY in all meetings
- Promote Y programs through public appearances and presentations

**Requirements**:
**EDUCATION/EXPERIENCE**:

- Bachelor's Degree in Health Promotion or Related Field or 3-4 years of experience;
- Bi-lingual or multi-lingual preferred
- Hold one national certification (ACE, ACSM, AFAA, NASM, NSCA, advanced YMCA certification), and 6 months previous practical/relevant experience;
- Current CPR/AED and First Aid certifications;
- Obtain additional required YMCA certifications/other certifications as needed;
- YMCA's Diabetes Prevention Program Lifestyle Coach
- LIVESTRONG at the YMCA Instructor
- Enhance Fitness Instructor
- Healthy Heart Ambassador
- Other based on available program
- Wear appropriate staff attire;
- Ability to bend, kneel, move, or stand for extended periods of time;
