**Behavioral Health Coordinator**

**Location**: Brooklyn, NY

**Schedule**:Full time Monday-Friday

As a Behavioral Health Coordinator for our mental health program, your primary responsibility will be reviewing patient charts to ensure comprehensive care delivery and adherence to treatment plans. You will collaborate with multidisciplinary teams to optimize patient outcomes and coordinate care transitions.

**Responsibilities**:

- Review patient charts to assess treatment progress and compliance.
- Coordinate with healthcare providers to develop and implement comprehensive care plans.
- Facilitate communication among team members to ensure holistic patient care.
- Monitor and evaluate patient outcomes, adjusting care plans as needed.

**Qualifications**:

- Bachelors degree in Social Work or related field.
- Strong understanding of mental health disorders and treatment modalities.
- Excellent communication and interpersonal skills for effective collaboration.
- Ability to work in a fast-paced environment with attention to detail and accuracy.

**Compensation**: Approximately $80,000 plus comprehensive benefits.

Note: Compensation is based on CPT codes

Pay: From $80,000.00 per year
