Overview
Provides direct psychosocial services to clients in the community who are at risk of homelessness or have frequent contact with the mental health system, substance use system, or criminal justice system. Assists in the overall functioning of the Intensive Mobile Treatment (IMT) Program. Works under general supervision.
Responsibilities
  • Performs social work assessments of clients; opens cases as appropriate.
  • Engages clients in natural settings to develop trustful relationships and support networks, in collaboration with the multi-disciplinary Intensive Mobile Treatment (IMT) team.
  • Collaborates with clients and multi-disciplinary IMT team to develop and implement individualized treatment plans.
  • Assists clients in the development of a sustainable network of community based supports, utilizing identified strengths and tools designed to prevent future participant crises and/or reduce the negative impact if a crisis does occur.
  • Prepares case presentations and maintains case records, which include activities, progress notes, evaluations, and recommendations, in accordance with the Program record keeping mechanism.
  • Maintains proper documentation and record of statistics in accordance with the Program's record keeping procedures.
  • Participates in interdisciplinary team meetings and case conferences.
  • Confers with clients in order to develop plans for post-treatment activities.
  • Coordinates and collaborates with other service providers on the treatment plans and client's needs, as well as follow up on linkages made between clients and other service providers to ensure continuity of care.
  • Understands and uses community resources and referral procedures linking families to needed services.
  • Assists and collaborates with the Program Manager in the overall functioning of the service with regard to growth, development and implementation of program goals.
  • Attends in-service education programs and other training, as required by the Agency and funding sources.
  • Participates in community programs and education, as requested.
  • Contributes to the formulation of clinical and administrative policies and procedures and the preparation of policy and procedure manuals, as required.
  • Participates in after hours on-call system.
  • Performs other related duties, as assigned.


Qualifications

Licensure: Valid driver's NY State license may be required, as determined by operational/regional needs.

Education: Master's Degree in Social Work, Counseling, or related degree required.

Experience: Minimum of one year as a social service provider experience with clients in a mental health setting required. Experience working in substance abuse treatment or mental health counseling or within the criminal justice system required. CASAC and/or other related trainings preferred. Bilingual skills may be required, as determined by operational needs.
