Direct Care Management D.B.A CareCollab is located in New York, NY. CareCollab specializes in coordinating comprehensive care for vulnerable populations struggling with chronic medical conditions, mental health issues, substance use disorders, and homelessness. These individuals often face fragmented and uncoordinated healthcare services.

Position Summary:
Objectives of this Role:

- Represent our company's service offering, starting with a comprehensive understanding of the health home program, the services we offer, and how members could benefit from enrollment in the program.
- Generate leads and build relationships by nurturing warm prospects and finding new potential referral sources.
- Manage and maintain a pipeline of interested prospects and engage the enrollment team for the next steps.

**Responsibilities**:

- Develop outreach strategy, content, and communication to generate qualified opportunities for enrollment.
- Deliver upon core enrollment metrics including internal and external account enrollment targets.
- Own and perfect the outreach and enrollment process: from initial prospecting to enrollment
- Identify the needs/challenges of the prospective referral sources and determine how CareCollab can be a solution for their agency and clients.
- Understand basic knowledge of target populations’ social, medical and behavioral health needs.
- Ability to relate to clients, community organization workers and leaders.
- Improve the outreach and enrollment process within internal partnerships.
- Develop all critical skills to accelerate a successful outreach and enrollment process.
- Ability to work independently, but seek assistance, when necessary, and provide suggestions for improvement.
- Provide regular updates regarding potential business development opportunities (internal and external accounts)
- Maintain HIPAA compliance in all activities and comply with all regulatory standards.

Required Skills and Qualifications
- A bachelor's degree is preferred.
- 2+ years of healthcare business management
- 2 + years of sales experience, with a history of exceeding lead targets
- Strong communication and presentation skills
- Proven creative problem-solving approach and strong analytical skills.
- Clean, professional image, behavior, and demeanor are always expected.
- Be self-driven with business development goals and the ability to execute plans to achieve desired outcomes.

Compensation/ Benefits
- Salary: From $75K Annually (Negotiable)
- Bonus from generated business
- 11 Days PTO + 7 Days Sick

Pay: From $75,000.00 per year
