Key Responsibilities\n- \n
- Conduct individual and family therapy sessions, both virtually and via video conferencing, to address the unique emotional needs of hospice patients. \n
- Develop and implement comprehensive care plans tailored to each client’s circumstances, focusing on psychosocial needs and effective interventions. \n
- Provide crisis intervention services, utilizing evidence-based approaches to support patients and their families during challenging times. \n
- Facilitate effective communication among clients, family members, and multidisciplinary team members to ensure seamless treatment delivery. \n
- Conduct thorough assessments, utilizing developmentally appropriate screening tools to inform care strategies and recommendations. \n
- Monitor patient progress through regular follow-ups, adjusting treatment strategies as necessary to align with evolving needs. \n
- Participate in regular team meetings to share insights, contribute to program improvements, and uphold agency goals of continuous quality improvement. \n
- Document all patient interactions and maintain accurate records to ensure compliance with ethical standards and organizational policies. \n
The primary focus of this role is to support individuals facing major mental illnesses and severe emotional disorders in a hospice setting. You will work closely with patients who are experiencing the emotional ramifications of life-limiting diagnoses, ensuring that their treatment is comprehensive, compassionate, and sensitive to their unique circumstances. Your goal will be to provide holistic support that addresses not just the clinical needs but also the emotional and social barriers to quality of life.
Qualifications\n- \n
- A Bachelor's Degree in Social Work (BSW) from an accredited institution is required. \n
- A valid and active LCSW license in the state of Arizona. \n
- A minimum of 2 years of clinical experience in a social work capacity, preferably within a mental health or hospice care setting. \n
- Strong interpersonal skills and the ability to build rapport with a diverse clientele, including patients and family members. \n
- Excellent written and verbal communication skills to facilitate effective documentation and team collaboration. \n

- Experience working in hospice care or with palliative care teams is highly desirable. \n
- Familiarity with electronic health record systems and data documentation processes. \n
- Background in crisis intervention and the ability to work independently as well as part of a multidisciplinary team. \n
The position offers a competitive salary ranging from USD 70,000 to 85,000 annually, commensurate with experience and qualifications.
