Work Environment

This position offers the flexibility of remote work, allowing the LCSW to provide services from home while still engaging actively with clients through telehealth platforms. As part of our hospice center, you will be a key member of a multidisciplinary team, collaborating with medical professionals, nurses, and family members to ensure comprehensive care. Although primarily remote, occasional visits to local facilities may be required to facilitate connections with clients and families directly.

Key Responsibilities
  • Conduct thorough and empathetic assessments to identify the social, emotional, and practical challenges faced by clients and their families in hospice care.
  • Develop personalized service plans that include specific goals, interventions, and strategies for progress monitoring, ensuring alignment with client needs and wishes.
  • Facilitate individual and family counseling sessions tailored to the unique psycho-social needs associated with end-of-life issues, grief, and loss.
  • Provide crisis intervention and risk assessment as necessary, guiding families through moments of heightened emotional distress.
  • Collaborate closely with clients to navigate community resources, including palliative care services, emotional support groups, and financial assistance programs.
  • Engage in community outreach to promote awareness of hospice services and the importance of comprehensive end-of-life care.
  • Participate in case conferences and team meetings, contributing insights and recommendations based on interactions with clients.
  • Maintain accurate and confidential case documentation in accordance with organizational policies and state regulations.
Patient Population / Client Focus

The Licensed Clinical Social Worker will primarily serve terminally ill patients and their families, offering support tailored to the emotional and psychological complexities that arise during hospice care. This includes addressing concerns related to terminal diagnoses, facilitating grief counseling, and assisting families in understanding the stages of the dying process. The LCSW will also be responsible for promoting a dignified end-of-life experience, advocating for patient rights, and ensuring that care aligns with the values and beliefs of each individual and family.

Qualifications
  • A Master’s degree in Social Work from an accredited program is required.
  • Current and active licensure as a Licensed Clinical Social Worker (LCSW) in Arizona.
  • Demonstrated understanding of hospice care principles, particularly regarding end-of-life issues and patient advocacy.
  • Strong communication skills to effectively support families during emotionally challenging times.
  • Proficiency in trauma-informed care practices and cultural competence in dealing with diverse populations.
Preferred Experience
  • Prior experience in a hospice or palliative care setting is highly desirable.
  • Experience working in social services, particularly in case management or counseling roles supporting individuals facing significant life challenges.
  • Knowledge of community resources specific to healthcare, end-of-life services, and bereavement support.
Compensation and Benefits
