Patient Navigator I:

Patient navigator II:

$24.25 - $29.47 Hourly / $1,940.00 - $2,357.60 Biweekly Bilingual incentive pay is +7.5% above the Patient Navigator I/II rate. Description: Under direct supervision, provides support and/or client services designed to ensure effective navigation of health services to minimize fragmentation within the healthcare system and maximize quality treatment for patients of the Health Services Agency. Serves as an advocate for clients to resolve concerns and improve patient access to services across all healthcare sectors. Collaborates with healthcare providers and multidisciplinary professional staff to implement safe and efficient patient care. May perform routine patient care in a clinical setting.

Examples of Duties

Assists patients in accessing care and navigating health care systems. Assess barriers to care and engage patients and caregivers in finding potential solutions. Schedules appointments for patients with various health care providers as directed. Coaches patients in self-management, problem solving and empower patients, family and/or caregivers to achieve maximum levels of wellness and independence. Bilingual Candidates:

I Education, Experience & Licenses

Any combination of training and experience which would provide the required knowledge, skills and abilities is qualifying. A typical way to obtain the required qualifications would be:

Equivalent to the completion of twelfth (12th) grade. Patient

Navigator

I : Two (2) years of experience working one-on-one assisting patients with their health needs. Must possess a clinical medical assisting certificate of completion from an accredited training institution or graduation from an accredited college or university with an Associate's Degree in a health or social services related field. Patient

Navigator

II : In addition to the above, must possess a valid certification as a Medical Assistant from an institution approved by the California Certifying Board for Medical Assistants. Requires one (1) year of related experience in a medical setting. Bilingual Candidates:

Must pass a Merit Systems Services written and/or oral bilingual proficiency test or other appropriate test selected by the County) to qualify. License and Special Requirements: Requires a valid California driver's license. Must complete periodic continuing education coursework as required to maintain active certification. Requires CPR certification. Other combinations of education and experience may be considered.

In addition, Tehama County is establishing an eligibility list to fill future vacancies for a Patient Navigator I/II - Bilingual or Patient Navigator I/II, as well as subsequent vacancies, which occur during the life of the eligibility list. SALARY:

Annual salary step increases based upon satisfactory performance and completion of one year of employment (Step A to E).

HEALTH:

Approximately 80% of the premium for medical, dental, and vision plan is paid by the County (EPO Plan). The plan covers the employee and all eligible dependents at NO ADDITIONAL COST Employee premiums are $234.54 per month as of January 1, 2025 for employee and all eligible dependents.

PAID LEAVES:

Benefits also include an annual paid leave plan of

96 hours of vacation (increases upon continuous years of service) 96 hours of sick leave 14 paid holidays plus one 8 hour personal holiday

LIFE INSURANCE:

Approximately 80% of the premium for a $30,000 life insurance plan for the employee is paid for by the County.

RETIREMENT:

A retirement plan through CalPERS.

CalPERS Formulas:

Classic member = 2% @ 55 New Member = 2% @ 62

A Deferred Compensation retirement plan option with a County match of up to a maximum of $60 per month.
