is part of the Oriol Health Care family owned and operated group of healthcare companies.

Responsibilities:

Performs assessment and documents in a timely fashion.
Develops individualized Plan of Care to be submitted for physician approval.

Evaluate the patient’s ADL and need for HHA; develops and implements HHA Plan of Care.
Coordinate community services for patient to assist in safe home care.
Effectively communicates with patient, family, physician, and other disciplines involved in the case.
Reviews billing process with patient/family.

This is a brief description of some not all required responsibilities.

Education:

Graduate of an accredited nursing program; Valid MA RN or LPN nursing License
1 year of experience is preferred but will consider a new graduate nurse.
