As a Home Health RN, you will play a vital role in our team, working closely with patients, families, and healthcare professionals to provide comprehensive care and support.

Job Overview

As a Home Health RN, you will be responsible for assessing patients' physical and psychosocial needs, developing personalized care plans, and delivering high-quality care in the home setting. You will work collaboratively with our interdisciplinary team to ensure seamless care transitions and optimal patient outcomes.

Responsibilities

  • Conduct thorough assessments of patients' physical and psychosocial needs
  • Develop and implement individualized care plans to meet patients' unique needs
  • Deliver high-quality care in the home setting, including wound care, medication management, and education
  • Collaborate with patients, families, and healthcare professionals to ensure seamless care transitions
  • Document patient progress and outcomes in accordance with regulatory requirements

Requirements

  • Current RN licensure in Massachusetts
  • Valid state-issued driver's license
  • One year of acute medical/surgical nursing experience
  • CPR certification
