The Outpatient Nurse Coordinator is a professional caregiver who assumes responsibility and accountability for a group of patients for a designated time frame and provides care to these patients via the nursing process and input from healthcare team members. The nurse will provide direct patient care and coordinate CCS Cardiac Special Center Care team services and other clinical nursing services in the Children's Heart Center. The nurse will coordinate patient/caregiver teaching, referrals and follow-up nursing care for patients, track patient compliance with visits and medical care recommendations; and coordinate multidisciplinary team conferences to review patients in the CCS Center Care program. The nurse will provide nursing support, clinical judgment, patient education, letter and form completion, phone triage and care coordination for Children's Heart Center patients. The nurse will work collaboratively with physicians, clinical staff, and administrative staff.

Salary range: $59.46 - $88.39 Hourly

Qualifications

  • BSN or MSN – required  
  • Active California RN license – required
  • ACLS, BCLS and PALS from the American Red Cross or American Heart Association – required
  • At least 6 months of clinical nursing experience – required
  • Ability to delegate tasks in a management style that is consistent with promoting leadership and teamwork
  • Knowledge base to provide education to patients, families, and other staff regarding clinical and patient care issues
  • Official Transcripts and/or original diploma from a registered nursing program must be provided upon hire
