Job Requirements:

Graduate of an accredited School of Nursing (LPN, RN or BSN) with a valid state License.
Three years of long-term care clinical nursing experience.
Specialized training in infection prevention and control programs preferred.
Knowledge of MDS/RAI requirements preferred.
Knowledge of QAPI and performance improvement processes preferred.
Strong computer, interpersonal, leadership, organizational and clinical skills.
Ability to communicate effectively with Members, their families and all levels of the organization.

Responsibilities:

Management of and Facilitation of the QAPI Program

Maintain documentation and demonstrate evidence of its ongoing QAPI program that meets the requirements of CMS Regulation (§483.75, F865). This may include, but is not limited to:
Facilitating systems and producing reports demonstrating systematic identification, reporting, investigation, analysis, and prevention of adverse events;
Documentation demonstrating the development, implementation, and evaluation of corrective actions or performance improvement activities;
Regularly monitors and distributes Quality Indicator/Quality Measures information as it relates to the MDS process and survey preparation;
Advises the Director of Nursing on clinical risk management and offers feedback, research, and education to lessen risks within the Health Care Households;
Perform documentation reviews, and provide notification to Health Care Household leaders when documentation compliance fails to meet standards; and
Presenting information to the State Survey Agency upon request.
Track all required In-Service education and manage employee education files for State Survey Agency purpose.

Coordination of Electronic Medical Record (EMR) Implementation and Usage

Responsible for coordinating, along with the Director of Nursing, EMR use within the Health Care Households:
Offers initial training and ongoing support to new staff members on the use of EMR;
Leads efforts to implement new EMR functions and modules; and
Serves as a resource for the State Survey Agency during all site visits, surveys or inspections.

Serves, as needed by special circumstance, as supplemental effort for the RAI Process

Responsible for the RAI process during special circumstances:
Vacancy of the MDS Coordinator Position;
Temporary absences of the MDS Coordinator position, such as training or illness; or
Increased RAI/MDS workload due to census, resident condition.
Temporary absences of the Move-In Coordinator Position.
Temporary absences of the Social Services Coordinator Position.

Infection Prevention and Control Program (IPCP)

Responsible for the surveillance, reporting, assessment and training related to infections:
Track vaccination status and report to NHSN routinely.
Develops, maintains, and updates annually, ICPC policies, procedures and practices;
Develops an antibiotic stewardship program that includes antibiotic use protocols and a system for monitoring antibiotic use;
Collaborates with DON to develops immunization policies and procedures and monitors the influenza and pneumococcal immunization process; and
Develops education or training related to IPCP.
Organize and manage all Infection Control files.

Other Duties and Special Projects

Provide education to residents on new diagnosis and treatment in coordination with Household Managers.
Conducts monthly Resident Council Meetings.
Complete PASRR for new residents.
As part of the team that works directly to manage and lead the Health Care Households, other duties may be assigned by the Director of Nursing. Also, attendance at certain meetings and participation in certain committees may be expected.

Additional Responsibilities

Meets all resident and household needs by actively assisting (to the extent of cross training and certification) in all tasks of the household.
Ensures that resident rights and responsibilities are honored and met.
Monitors the community and household response to resident concerns through continuous quality improvement.
Maintains professional presence when representing Trinity Woods; exhibits degree of professionalism consistent with standards in performance, behavior, and appearance.
Demonstrates professional development/skills competency in all activities.
Complies with Trinity Woods’ established safety rules and practices.
