OBJECTIVE:


To oversee and coordinate all aspects of the Infection Prevention and Control Program, ensuring compliance with the Centers for Medicare and Medicaid Services (CMS) standards. This role encompasses various responsibilities aimed at enhancing patient safety and quality of care.

Actively participates in antibiotic stewardship initiatives and contributes to the Quality Assurance Performance Improvement (QAPI) Committee.

REPORTING STRUCTURE:
Director of Nursing

EMPLOYMENT STATUS:
Must maintain at least part-time employment with the facility.

CONFIDENTIALITY:
Responsible for adhering to HIPAA MINIMUM NECESSARY guidelines to protect sensitive health information.

WORK ENVIRONMENT:
Well-lit and ventilated office and clinical areas. The role may require standing, bending, and walking intermittently. Must be prepared to handle emotional situations involving residents, their families, and staff. Exposure to infectious diseases and odors may occur. Compliance with the No Lift Policy is mandatory.

KEY RESPONSIBILITIES:
1. Uphold and enforce the Infection Control and Prevention Program established by White Oak Management.
2. Set benchmarks and assess infection prevention practices and outbreak responses within the facility.
3. Facilitate and supervise staff training during orientation, annually, and as necessary.
4. Provide education to residents, families, and visitors as required.
5. Identify and document infections through a systematic surveillance process.
6. Assess the need for and implement appropriate transmission-based precautions.
7. Report communicable diseases to the public health department.
8. Support and manage the Antibiotic Stewardship Program and its initiatives.

Collaborate with various departments to minimize cross-contamination risks and enhance infection prevention strategies.

Develop action plans to address identified areas for improvement in staff and facility practices.
Coordinate employee health matters with human resources and departmental leaders.
Conduct the annual Infection Control Risk Assessment for the facility.

Engage with community partners (e.g., mental health facilities, hospitals, assisted living centers, and other long-term care providers) to mitigate infection transmission risks across the care continuum.

Perform regular infection prevention and control rounds throughout the facility.

Work closely with licensed nursing staff and attending physicians to ensure adherence to established protocols regarding antibiotic therapy initiation.

Prepare and present infection prevention and control and antibiotic stewardship reports on a monthly, quarterly, and annual basis to the QAPI Committee for evaluation and feedback.

Monitor schedules for routine cleaning of the facility, including terminal room cleaning and equipment sanitation.
Oversee staff competencies related to infection prevention and control during hiring, annually, and as needed.
