OBJECTIVE:


To oversee and manage all aspects of the Infection Prevention and Control Program, ensuring compliance with the standards set by the Centers for Medicare and Medicaid Services (CMS). This role encompasses various responsibilities aimed at enhancing patient safety and quality of care.

Actively participates in antibiotic stewardship initiatives and serves as a vital member of the Quality Assurance Performance Improvement (QAPI) Committee.

REPORTING STRUCTURE:
Reports directly to the Director of Nursing

EMPLOYMENT STATUS:
Must maintain at least part-time employment within the facility.

CONFIDENTIALITY REQUIREMENTS:
Must adhere to HIPAA guidelines and ensure the protection of sensitive health information.

WORK ENVIRONMENT:
Office areas and patient rooms that are well-lit and ventilated. Responsibilities may include standing, bending, and walking throughout the day. The role may involve addressing emotional challenges faced by residents and their families, as well as potential exposure to infectious agents and odors. Compliance with the No Lift Policy is mandatory.

KEY RESPONSIBILITIES:
1. Uphold and enforce the Infection Control and Prevention Program established by White Oak Management.
2. Set benchmarks and assess infection prevention practices and outbreak responses within the facility.
3. Provide and supervise educational sessions for staff during orientation, annually, and as needed.
4. Facilitate educational initiatives for residents, families, and visitors as required.
5. Identify and document infections through a systematic surveillance process.
6. Assess the necessity and type of precautions to be implemented or lifted.
7. Inform public health authorities regarding reportable infections.
8. Support and manage the Antibiotic Stewardship Program and its initiatives.

Collaborate with various departments to minimize cross-contamination risks and enhance infection prevention measures.

Develop action plans aimed at improving staff and facility practices.
Coordinate employee health issues with human resources and department leaders.
Conduct the annual Infection Control Risk Assessment for the facility.

Engage with community partners, including mental health facilities, hospitals, and other long-term care providers, to mitigate infection transmission risks across care settings.

Conduct regular infection prevention and control inspections throughout the facility.

Work closely with licensed nursing staff and attending physicians to ensure adherence to established protocols for initiating antibiotic treatments.

Prepare and present infection prevention and control and antibiotic stewardship reports on a monthly, quarterly, and annual basis to the QAPI Committee for review and feedback.

Monitor schedules for routine cleaning of the facility, including terminal cleaning and equipment sanitation.
Oversee staff competencies related to infection prevention and control during onboarding, annually, and as needed.
