Reports to: Clinical or Program Director as Assigned

_*Position Summary: Counselor is a clinical position providing residential treatment services to clients. As such, this position works within the framework of a multi-disciplinary team, counsels and advises individuals with substance abuse and / or other problems related to mental health as determined by the medical staff.*_

*Primary Responsibilities*

While managing a caseload:

* Perform clinical assessment of incoming patients and create an effective treatment plan.
* Counsel patients, individually and in group settings, to assist with overcoming dependencies, adjusting to life or making changes.
* Document in a timely and accurate manner, records and notes on all patient activity and communications on patient’s behalf in electronic medical record
* Advocate, collaborate and communicate with community partners on behalf of patients regarding clinical record and discharge planning.
* Maintain the highest level of patient confidentiality in accordance with Awakenings confidentiality agreement, HIPAA requirements and CFR 42, Part II.
* Recognize and set patient care priorities and coordinate activities in support of clinical and administrative staff.
* Provide quality patient care in collaboration with multi-disciplinary clinical team.

_*And any other duties as assigned by your direct supervisor*_

*Position Requirements*

* Minimum CADC I and currently certified in Oregon State or BA/Masters working towards licensure.
* Maintain and keep current required clinical certifications.
* Individuals recovering from any substance abuse-related disorder(s) must be able to demonstrate to date, continuous sobriety of at least two continuous years.
* Understanding of appropriate patient documentation as required by agency standards and the state of Oregon for residential treatment facilities.
* Current CPR and First Aid certifications.
