I) Job Purpose

A) Reporting to the Clinic Practice Manager at Access to Care, the Clinical Therapist completes diagnostic assessments or therapy sessions per agency demand. Activities include completing the initial diagnostic assessment, ANSA, and Person-Centered Wellness Plan at Access to Care and in the adult outpatient clinics. Activities also include engaging individuals into services, providing resources, crisis intervention, and other formal and informal assessments.

II) Essential Functions

A) Performs face to face assessment as part of the initial eligibility determination process for individuals seeking services from MHMR Tarrant. This includes evaluation of diagnostic eligibility according to target population guidelines.

Performance Standards:

1) Complete the initial assessment, including assigning a provisional diagnosis, on all assigned individuals 100% of the time.

2) Makes appropriate referrals for follow-up care 100% of the time.

B) Performs Mental Health Adult ANSA and Clinical Rating Scales for assigned clients.

Performance Standards:

1) As directed by Program Manager and according to program need, completes ANSA and Clinical Rating Scales, including process and documentation

2) Maintains knowledge and training needed to complete these processes in compliance with all applicable requirements.

C) Completes a Person-Centered Wellness plan that reflects the strengths, needs, abilities, and preferences of individuals for their recovery.

i) Performance Standards:

(a) Completes Wellness Plans as part of the intake process as director by Program Manager at Access to Care.

D) Provides SBIRT services as program demands.

Performance Standards:

1) Successfully completes referrals for SBIRT with appropriate follow up 100% of the time.

E) Provides CBT/CPT services as program demands.

Performance Standards:

2) Successfully completes referrals for CBT and/or CPT with appropriate follow up 100% of the time.

F) Maintains current licensure, and practices in accordance with standards set by Texas Board of Social Work Examiners, Texas State Board of Examiners of Marriage and Family Therapists or the State Board of Examiners for Licensed Professional Counselors 100% of the time.

G) Provides crisis intervention/resolution and develops plan for follow-up care.

Performance Standards:

1) Assesses and documents suicidal and homicidal ideation and intent 100% of the time.

2) Arranges clinically appropriate care and follow-up for clients who have suicidal and homicidal ideation and intent 100% of the time

3) Implements triage procedures, including conferring with other licensed staff, and informs Program Manager of imminent dangerousness of clients immediately, 100% of the time.

H) Thoroughly documents treatment provided to clients.

Performance Standards:

1. All documentation is present in chart/data system within 24 hours of service provided.

2. All documentation is completed according to MHMRTC policies and procedures and Texas Health and Human Services Community Standards.

I) Exhibits a willingness to assume additional duties; seeks the guidance of supervisor prior to beginning an unfamiliar assignment.

J) Regular attendance is required.

K) Worksite may be reassigned to meet program needs based on management authority.

L) Performance standards are performed as applicable with MHMR’s We CARE values “We Connect People in Our Community. We Provide Access to Services. We Link People to Resources. We Empower People.”

M) Perform other job duties or responsibilities as requested or assigned.

III) Knowledge of Laws, Regulations, Policies/Procedures, Skills, and Abilities

A) Demonstrated skill in developing productive relationships with clients, family members, service providers and professional peers and subordinates.

B) Extensive knowledge of the needs and characteristics associated with individuals who are in the target population.

C) Extensive knowledge of case management, habilitation and rehabilitation principles, objectives, standards and methods.

D) Extensive knowledge of the current DSM criteria and disorders.

E) Ability to work independently and organize work effectively and to prioritize workload demands.

F) Ability to conduct screenings, referrals, intakes, assessments, and unit orientations.

G) Excellent written and oral communication skills.

H) Ability to complete proper documentation/paperwork required for individual chart.

Prior approval from supervisor is required for all Paid Time Off (PTO) and Overtime.
