Albany Medical Center is seeking a compassionate and skilled Nurse Practitioner to join our Palliative Care team. This is an opportunity to provide holistic, patient- and family-centered care in the Capital Region’s only academic medical center as part of our adult consult service.About the RoleAs part of our interdisciplinary palliative care team, the APP will provide specialized medical care for patients living with serious or life-limiting illnesses. The focus is on symptom management, quality of life, and support for patients and their families through all stages of illness, including end-of-life care.Clinical ResponsibilitiesConduct comprehensive assessments of patients with complex medical needs, including pain, symptom burden, psychosocial concerns, and spiritual care needs.Develop, implement, and adjust individualized care plans in collaboration with attending physicians, nurses, social workers, and chaplains.Provide expert symptom management for issues such as pain, dyspnea, nausea, anxiety, and delirium.Facilitate advance care planning discussions and assist patients and families with understanding goals of care and treatment options.Participate in family meetings to ensure alignment between medical recommendations and patient preferences.Provide palliative care consultations for hospitalized patients and assist with transitions of care to home, hospice, or other settings.Educate patients, families, and healthcare team members on palliative care principles.Contribute to quality improvement initiatives and participate in palliative care education for traineesQualificationsCurrent NYS licensure as a Nurse Practitioner (or eligibility)National board certification as an NPExperience in palliative care, oncology, geriatrics, or critical care preferredStrong communication skills, empathy, and ability to work collaboratively within a multidisciplinary teamCommitment to patient- and family-centered careCompetitive base salary range: $126,000-$147,000 commensurate with experience.Albany Medical Center is the centerpiece of medicine, research, and medical education in New York’s Capital Region and is the area's largest private employer with more than 10,000 employees.Albany Medical Center offers excellent career opportunities in a wide range of roles in both patient care and administration.
