Palliative Care Physician Assistant

Albany Medical Center is seeking a compassionate and skilled Physician Assistant to join our Palliative Care team. This is an opportunity to provide holistic, patient- and family-centered care in the Capital Regions only academic medical center as part of our adult consult service. About the Role As part of our interdisciplinary palliative care team, the APP will provide specialized medical care for patients living with serious or life-limiting illnesses. The focus is on symptom management, quality of life, and support for patients and their families through all stages of illness, including end-of-life care. Clinical Responsibilities Conduct comprehensive assessments of patients with complex medical needs, including pain, symptom burden, psychosocial concerns, and spiritual care needs. Develop, implement, and adjust individualized care plans in collaboration with attending physicians, nurses, social workers, and chaplains. Provide expert symptom management for issues such as pain, dyspnea, nausea, anxiety, and delirium. Facilitate advance care planning discussions and assist patients and families with understanding goals of care and treatment options. Participate in family meetings to ensure alignment between medical recommendations and patient preferences. Provide palliative care consultations for hospitalized patients and assist with transitions of care to home, hospice, or other settings. Educate patients, families, and healthcare team members on palliative care principles. Contribute to quality improvement initiatives and participate in palliative care education for trainees Qualifications Current NYS licensure as a Physician Assistant (or eligibility) National board certification as an PA Experience in palliative care, oncology, geriatrics, or critical care preferred Strong communication skills, empathy, and ability to work collaboratively within a multidisciplinary team Commitment to patient- and family-centered care Competitive base salary range: $126,000-$147,000 commensurate with experience.
