Palliative Care Navigator

Palliative Care Navigator Job Description
ChristianaCare is seeking a skilled Palliative Care Navigator to join our team in Newark, Delaware. As a Palliative Care Navigator, you will play a vital role in coordinating care for patients and their families, ensuring they receive the best possible support and treatment.
Key Responsibilities:
- Coordinate referrals from within and outside of Christiana Health Care
- Assist Palliative Care staff with coordinating care and facilitating referrals
- Identify potential and realized barriers to care and facilitate referrals
- Schedule and/or facilitate scheduling of appointments
- Triage of family/caregiver phone calls and messages
- Coordinate response with Palliative clinicians based on clinical assessment
- Organize inpatient palliative census and daily workload
- Evaluate and triage consults by collaboration with the hospital team
Requirements:
- BSN required
- Masters in nursing preferred
- RN license or eligible for licensure in the state of Delaware
- Three or more years of combined clinical experience in clinical and/or administrative experience in case management, clinical trials, older adult, palliative/hospice care preferred
