Job Description:

Patient Care Coordinator: under the direction of the Department Director and/or Charge Nurse utilizes a variety of clinical and leadership skills to educate, consult, and facilitate the service-line team in achieving the highest quality of service and satisfaction for the patients. We are looking for a Patient Care Coordinator who will be responsible for providing patient-centered care in a clinical setting. This role requires a high level of expertise in nursing practice, as well as excellent interpersonal and communication skills. The ideal candidate will be a compassionate and dedicated healthcare professional who is committed to delivering high-quality care to patients.

Responsibilities:

  • Coordinates and participates in a variety of administrative and clinical activities.
  • Provides a clinical and psychosocial approach through a continuum of care and balances individually identified patient and family needs with efficient and cost-effective utilization of resources.
  • Coordinates and participates in a variety of administrative and clinical activities.
  • Coordinates care, educating patients, and building a trusting relationship between the patients and the medical care team, including but not limited to assessing, planning, directing, implementing, coordinating and counseling as needed.

Requirements:
  • Minimum of 3 years of recent experience as an RN related in a clinical setting or minimum of 5 years of recent experience as an LPN related in a clinical setting.
  • Active license within the State of practice.
  • Current BLS and ACLS certifications
  • Associate or Bachelor Degree in Nursing
