*Position Summary – After Care Coordinator*

We are currently recruiting for an After Care Coordinator to be responsible for providing essential non-clinical support during all phases of follow-up after inpatient services. This role is ideal for an organized and detail-oriented individual with strong communication skills who is dedicated to ensuring that members receive the necessary follow-up care to support their recovery and well-being.

Key responsibilities include:
* Following up with members identified by clinical operations as needing aftercare support.
* Verifying compliance with initial follow-up appointments and assisting with referrals if appointments are missed.
* Coordinating outpatient follow-up care and checking provider availability.
* Documenting results of follow-up calls in the system and completing non-clinical assessment tools related to aftercare and HEDIS activities.
* Contacting members and providers to gather discharge and other non-clinical information, following established scripts and workflows.
* Developing and completing reports on outstanding aftercare activities for internal use and client requests.
* Supporting quality management staff with non-clinical tasks related to HEDIS expectations and other quality initiatives.

*Requirements:*
* High school diploma or GED required; BA/BS in a health-related field preferred.
* Experience in customer service or a related field.
* Proficiency with office software such as Word, Excel, and PowerPoint is preferred.

*Compensation and Benefits*

Salary
* *$16 - $18 per hour*, commensurate with experience
