This work stems from Mass General Brigham's unified system strategy to bring high-quality healthcare closer to patients while lowering total healthcare costs. MGBMG provides a wide range of offerings, including primary care, behavioral and mental health, specialty care, outpatient surgery, endoscopy, imaging, lab services, infusion, and urgent care both digitally and in person in Massachusetts and New Hampshire.
As a hybrid opportunity, the position entails onsite coverage 4 days per week (days can be flexible depending on the candidate's schedule) and remote coverage 1 day per week. Due to the hybrid nature of the role, we ensure that our employees receive the required technology and training to be proficient and independently productive in all job responsibilities regardless of work location. Employees are responsible for designating a workspace within the remote work location that is private, safe, ergonomic, and free from distractions for all hours worked.
Our Clinical Social workers are integral members of the ambulatory primary care team, working closely with the clinical and administrative staff to enhance the delivery of patient care services along the continuum of care. This is a collaborative approach to providing the best care This employee will identify high-risk psychosocial factors of patients, help primary care staff understand the influence of those factors upon the course of medical care, and assume care coordination responsibility of the patients. In other cases, the social worker will be working with the treating outpatient clinicians (in or outside of our system) helping to ensure the treatment is focused, effective, and carried through.
