Works closely with other nutritionist/dietitians, interdisciplinary care team members, residents, registrants and their families, management and other departments and department heads to provide proper nutritional care.

Position Summary: As per LISVH policies and the ethical standards of Dietetics, the Dietician will oversee advisory, counseling, and instructional services aimed at making recommendations and resolving nutritional issues. The primary objective is to assist the facility in meeting residents' nutritional requirements and ensuring optimal care through the Department of Food & Nutritional Services.

Required Qualifications:

Bachelor's degree with a major in Nutrition, Dietetics, or related field

Current Registration or Registry eligible with CDR Certification

NYS Certified Dietician

Thorough knowledge of Medical Nutritional Therapy and Long Term Care documentation requirements

Excellent interpersonal and communication skills with the ability to collaborate effectively within a multidisciplinary team.

Must be able to speak, write, read and understand English fluently

Ability to meet travel requirements associated with this position.

Ability to meet acceptable background check standards.

Ability to work flexible hours, including evenings and weekends, as appropriate.

Preferred Qualifications:

Master's degree in Dietetics, Nutrition, or related field

1+ year in Long Term Care

Member of the Academy of Nutrition and Dietetics

Member of Long Island Dietetic Association

Position Responsibilities:

Maintain open lines of communication with the Department of Food & Nutritional Services to ensure compliance with the Department of Health, Veterans Administration, and OBRA codes and regulations to meet the resident's nutritional requirements

Monitor and consult with Director of Nutritional Services regarding preparation of resident's food services, correct temperature, palatability, portion control, appearance, accuracy, and resident satisfaction of meals via meal observations and CQI results.

Attend all resident care plan meetings and will subsequently complete the required nutritional assessments, care plans, progress notes, and interim notes for those residents assigned to his/her charge. The frequency is determined per LISVH policies and procedures and the Department of Health specifications.

Will make recommendations related to resident care plans in the areas of but not limited to: fluid, electrolyte, vitamin, mineral, carbohydrate, protein, fat, and caloric needs.

Conduct comprehensive nutritional assessments of residents upon admission and regularly thereafter.

Develop individualized dietary plans based on assessment findings, medical history, and dietary preferences.

Monitor and evaluate residents' nutritional status and adjust dietary plans as necessary.

Provide ongoing nutrition education and counseling to residents, families, and caregivers.

Collaborate with nurses and other healthcare professionals to optimize resident care plans.

Contribute to the discharge care planning process including completion of all discharge summary forms and attendance of schedule discharge meetings.

Attend Interdisciplinary Comprehensive Care Plan meetings and other resident oriented meetings as necessary.

Participate in interdisciplinary team meetings to discuss resident progress and care goals.

Ensure compliance with regulatory standards, including federal, state, and facility-specific guidelines.

Oversee the implementation of therapeutic diets and ensure proper documentation of dietary interventions.

Ensures meals services meet the individual requirements of each resident specific nutritional needs.

Develop individualized, comprehensive care plans, document appropriately and input care plan data into the computer system.

Mentoring Dietetic Interns as assigned

Stay current with developments in the field of dietetics and incorporate evidence-based practices into clinical care.

All other duties as assigned by Director/supervisor.
