Palliative Nurse

Full-Time 12265 Townsend Road Nursing RN/LPN Day Eligible for $6k Sign On Bonus Redeemer Health Home Care and Hospice is seeking an experienced Palliative RN to join our team in providing quality, compassionate care for our patients and their families. This Palliative RN position will primarily provide patient services in Eastern Montgomery County/Parts of Philadelphia and is eligible for a $6k sign on bonus. SUMMARY OF JOB: A Palliative Visiting RN provides safe, holistic care to a population with multiple co-morbidities and advanced chronic illnesses. The RN educates regarding self management and provides support to meet the needs of the patient, family, and community. The RN coordinates the overall care plan for each patient, flexing between home health and hospice programs to assure continuity of care across the continuum from acute to chronic to end of life care. A primary way staff connect to our mission is by collaborating with all team members in promoting an environment that provides quality, compassionate, holistic, patient-centered care and supports all internal and external customers RECRUITMENT REQUIREMENTS: Minimum of 1 year med-surg experience required; 1 year of home care experience preferred; BSN preferred; Cardio/Pulmonary/Oncology experience preferred; able to work weekend, holiday, and on-call hours; IV and venipuncture experience preferred; able to demonstrate professional competencies in a home care setting; able to efficiently utilize technology (cell phone, computer); able to travel in service area. LICENSE AND REGULATORY REQUIREMENTS: Current State RN License, Valid Driver's License, Current CPR Certification EOE
