Redeemer Health Home Care and Hospice is seeking an experienced Palliative RN to join our team in providing quality, compassionate care for our patients and their families. This Palliative RN position will primarily provide patient services in Eastern Montgomery County/Parts of Philadelphia and is eligible for a $6k sign on bonus. medical and dental insurance eligible the first of the month following start date

Palliative RNs enjoy a flexible schedule, autonomously providing 1:1 direct care in patient homes with the support of an encouraging and experienced clinical team.
SUMMARY OF JOB: A Palliative Visiting RN provides safe, holistic care to a population with multiple co-morbidities and advanced chronic illnesses. The RN educates regarding self management and provides support to meet the needs of the patient, family, and community. The RN coordinates the overall care plan for each patient, flexing between home health and hospice programs to assure continuity of care across the continuum from acute to chronic to end of life care.

1 year of home care experience preferred; Cardio/Pulmonary/Oncology experience preferred; able to work weekend, holiday, and on-call hours; able to demonstrate professional competencies in a home care setting; able to efficiently utilize technology (cell phone, computer); able to travel in service area.
