Description
The RN Stroke Navigator will be the first contact the patient before hospital discharge and then follow the patient after discharge providing coordination between the patient, PCP, neurologist and other specialists. This position will perform in hospital early post discharge screening for medication compliance, healthy life style, depression, medical complications, and caregiver burden while all the while being the patients advocate and anticipating their needs.

Required Qualifications:
- Bachelor's Degree - Nursing; or obtained within 3 years.
- Oregon Registered Nurse License upon hire (Vendor Managed)
- National Provider BLS - American Heart Association upon hire.
- 5 years - Related neurological clinical experience.
Preferred Qualifications:
- Master's Degree - Nursing.
- NIHSS Certification upon hire.
- Stroke Certified Registered Nurse upon hire.
- American Association of Neuroscience Nursing membership and certification upon hire.
- 1 year - Experience in an Ambulatory setting.
- Experience working with patients who have neurological conditions, and their families.
- Experience working with a Joint Commission Certified Stroke Center.
- Experience leading formal educational forums/in-services.

The amounts listed are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
