  • Providing support services to patients in their homes under the supervision of the Hospice RN.
  • Assisting the patient in the achievement of physical and emotional comfort according to the Plan of Care
  • Help with therapeutic exercise, light household tasks, and ADLs
  • Recognize and report changes in condition to the RN
  • Monitor and report changes in patient condition to the registered nurse or hospice team members.
  • Support patients and families in coping with the challenges of terminal illness, providing compassionate companionship and emotional support.
  • Assist with documentation and maintaining accurate records of patient care activities.

Candidate requirements

  • High school diploma or equivalent required; completion of a state-approved Certified Nursing Assistant program.
  • Current certification as a Nursing Assistant (CNA) in the state of Oregon.
  • Minimum of 1-2 years of experience in a healthcare setting, preferably in hospice or palliative care.
  • Compassionate and empathetic demeanor with a genuine desire to support patients and families during end-of-life care.
  • Must possess a valid driver's license and reliable transportation for home visits.
  • Certification in Basic Life Support (BLS) is preferred.
  • Excellent communication and interpersonal skills, with the ability to interact sensitively with patients and families from diverse backgrounds.
  • Ability to work collaboratively as part of a multidisciplinary team, including nurses, social workers, chaplains, and other healthcare professionals.
