Duties:Utilizes substance abuse and mental health disorder knowledge to develop an integrated diagnosis. Selects proper treatment course based on for severity, lethality risk, motivation/readiness for treatment. Collaborates with client to develop treatment plans. Identify and prioritize problems and goals. Follows evidenced based therapy and culturally accepted practices when working with clients. Provides individual, family, and group counseling. Educations client and families with information regarding substance abuse, co-occurring disorders, treatment, recovery and resources. Works with clients to develop tools that can help them to manage anxiety, distress and emergencies as needed. Consults with other members of the health team regarding significant changes in patient status as needed. Participates in multi-disciplinary treatment planning. Completes accurate progress notes in electronic health record within 24 hours of service. Advocates for patient health care needs. Completes required continuing education and other requirements to maintain professional licenses and certificates. Participates in meetings to integrate patient care. Applicant must have computer proficiency. QUALIFICATIONS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. EDUCATION and/or EXPERIENCE: Master's degree in psychology, counseling, addictions, social work or other human services related field. Must have 5 years working in a community health organization. Must have at least 3 years experience working in a community health organization preferred. CERTIFICATES, LICENSES, REGISTRATIONS: Must possess a current and unrestricted license as a LMHC, LPC, LCSW, LMFT in addition to SUDP or MAC. Must have a valid Driver's License. Current TB Test, CPR/First Aid and HIV/AIDS/STD certification/training required. Must have BLS (Basic Life Support) Certification. Knowledge of and familiarity with ICD 10 and CPT codes required.
