  • $1,000 will be awarded in the first paycheck.
  • $1,250 will be awarded upon completion of the six (6) month interim performance evaluation with an overall rating of "Meets Expectations".
  • $1,750 will be awarded upon completion of the employee's one (1) year probationary period and a performance evaluation with an overall rating of "Meets Expectations".

The City of Virginia Beach Department of Human Services is seeking to hire motivated candidates who have experience providing case management services to adults diagnosed with mental health and or substance abuse in a Behavioral Health setting for our BH Case Management program.

Responsibilities:
  • Provide clients with enhanced care coordination with equal focus on both behavioral and primary health in a mental health, *intellectual disability, or substance abuse setting.
  • Complete complex assessments, treatment planning, and crisis planning.
  • Provide clinical intervention, consultation, supportive counseling, transition planning and other case management duties.
  • Will meet with assigned clients within five days of receiving a case management referral.
  • Schedule a face to face appointment within five work days with any client coming out of a State or local hospital following an involuntary or voluntary admission.
  • Will attempt a collateral or face-to-face contact within five work days after a consumer's release from jail.
  • Complete clinical documentation and assures that all filing of clinical information and submission of billing information are done accurately and within the time frame required.
  • Complete and final approve progress notes in the Electronic Health Record (EHR) within 24 hours of service being provided.
  • May serve specialized populations and/or serve in specialized roles.
  • Will provide medically hospitalized clients with support with discharge planning.
  • Link with hospital staff and natural supports as appropriate, and will obtain medical records to ensure timely follow-up and quality coordination of care.
  • Provide leadership and training to team members on an as needed basis.
