The City of Virginia Beach Department of Human Services is seeking to hire a Clinician II with case management experience working with individuals experiencing substance use and serious mental health disorders for our BH Permanent Supportive Housing Program.

Duties:

* Perform clinically professional work assessing client needs.
* Develop, implement, and monitor service delivery and assistance plans.
* Act as an advocate and liaison for participants in securing housing, entitlements, treatment, medical care, and other community services and supports.
* Coordinate and monitor services with other agencies.
* Provide counseling to clients and assists them.
* Maintain records and files, prepare reports, and perform other related work as apparent or assigned to clients in a Permanent Supportive Housing (PSH) program.
* Utilize Critical Time Intervention (CTI) model to achieve.
* Provide CTI case management services to assigned caseload to include engagement and rapport-building.
* Refer and connect with appropriate services including long-term mental health and natural supports.
* Monitor agreed upon support plans, and transitioning of services to long-term services, as needed.
* Collaborate and coordinate with Permanent Supportive Housing Team including Housing Specialists and Peers on a frequent basis.
* Complete complex assessments, treatment planning, crisis planning, clinical intervention, consultation, supportive counseling, transition planning and other case management duties for the Permanent Supportive Housing Program utilizing the CTI evidence-based model.
* Complete clinical documentation and assures all filing of clinical information and submission of billing information are done accurately and within the time frame required.
