Our Behavioral Health Services programs provide residential and outpatient treatment for those struggling with mental illness, developmental disabilities, or substance use disorders. For many individuals, our programs literally save lives.

Summary/Objective:
The Case Manager provides support, advocacy, direction and guidance to families living in the shelter to achieve the goal of permanent housing. They are responsible for providing comprehensive assessments, in order to assist families in navigating and accessing resources to achieve social, economic stability, and to secure affordable and sustainable housing. Working as part of a multidisciplinary team and works closely with the other Case Manager, the Director, the Housing Specialist and Stabilization Specialist to meet the goals of each family and the program. Weekends, Holiday and shift rotation is required. The work includes, but is not limited to, identifying and addressing barriers which may prevent families from obtaining permanent housing and developing an individualized plan for each family/individual in which those barriers are addressed. The Case Manager is responsible for networking effectively with family members, physicians, courts, human service and community agencies and any other relevant persons or entities in order to promote the health and wellbeing of the family.

Essential Functions
  1. To develop and maintain a warm, friendly, professional, cooperative atmosphere in the shelter in which families live and staff work.
  2. Safeguard the privacy and confidentiality of the residents and staff and to treat all individuals with dignity and respect at all times.
  3. Provide intensive case management and assessment that includes review of all pertinent program documents, rule and guidelines with family.
  4. Support and maintain a caseload of 13-16 families, conducting interventions as needed to attain positive outcomes.
  5. Establish Department of Housing Community Development (DHCD)-mandated self-sufficiency plan with incoming families through Rehousing Rehabilitation Housing Plan (RRHP) and ETO software.
  6. To accompany families to community venues related to wellbeing of family and housing search and placement.
  7. Maintaining appropriate records pertinent to individualized plan for each family and other records as necessary and as directed by the Program Director.
  8. To assist the shelter in meeting the DHCD benchmark for rehousing families on an annual basis. The current benchmark is to rehouse 26 families every year. Meeting with all families on a weekly basis to review case management plans.
  9. Assist families in accessing appropriate services and resources for health care, career development, education and financial planning, housing etc.
  10. To attend staff meetings, management meetings, supervision meetings and other meetings or trainings.
  11. To establish and maintain professional, courteous communication at all times with all families, co-workers, colleagues, community representatives, medical personnel, legal personnel, DHCD personnel, and all others involved in the families' lives.
  12. Other duties as assigned.

Competencies
  1. Excellent verbal communication, written communication and interpersonal skills.
  2. Knowledge of homeless and housing issues, including housing subsidies and community resources.
  3. Must be able to develop professional relationships with court personnel, counselors, medical personnel, landlords, property managers, and other community personnel to promote the health and wellbeing of families and assist to secure and maintain permanent housing.
  4. Must be able to work sensitively and effectively with people of diverse cultural backgrounds.
  5. Ability to establish and maintain strong collegial relations with co-workers and service providers.
  6. Good computer literacy. Using ETO/Assist program knowledge.
  7. Demonstrates a customer focus.
  8. Demonstrates team spirit in carrying out responsibilities.
  9. Possesses a work ethic that supports agency policies and goals.

Work Environment
This position operates in an office environment, use of a computer, phone, and offsite visits to community venues.

Physical Demands

This Position requires lifting 10-20lbs, able to maintain a clean work environment, walk upstairs and have a valid drivers license.

Position Type/Expected Hours of Work
General Hours 3 pm to 7 pm, Monday to Friday. Flexible schedule to work with the resident's schedule.

Required Education and Experience
  1. Minimum: High school diploma or equivalent, 3+ years of experience working in case management, housing, homelessness or related field.
  2. Preferred: BA or BS in Human Services, sociology, or related field along with one year of experience working in case management, housing, homelessness or related field.
  3. CPR/First Aid Certification (within 90 days of hire)
  4. Preferred Bilingual English/Spanish, English/Creole
  5. Valid Driver's License
