Job Description
A.Medical Education
1.Work with patients to review treatments suggested by the clinician
2.Provide new start education for all new therapies including administration
3.Review side effects
4.Provide medication reconciliation and drug interaction screening
5.Assist with adherence longitudinally
6.Serve as a point person for all medication related questions and concerns from patients
B.Education Responsibilities:
1.Assist in submitting prior authorizations
2.Support appeals for denied therapies
3.Learn the specialty pharmacy landscape and partner with our specialty pharmacy
4.Maintain a database of prior authorizations (PA) to ensure reauthorizations are completed prior to PA expiration
5.Work with Patient Assistance Program team to collaborate and monitor patients obtaining medications via patient assistance programs
6.Work with formulary committee and EPIC team when new therapies are approved
7.Process prescriptions in outpatient pharmacy to ensure timely access to new and refill prescriptions for patients in the IBD clinic
C.Financial assistance
1.Partner with social work and finance team on insurance needs and lapses
2.Provide critical support to a team evaluating the revenue from the IBD patient base
D.Monitoring
1.Partner with social work and finance team on insurance needs and lapses
2.Provide critical support to a team evaluating the revenue from the IBD patient base
E.Health maintenance
1.Provide vaccines
2.Assist with smoking cessation
3.Making necessary referrals to other clinicians as needed
F.Multidisciplinary collaboration
1.Engage with members of the care team including those from the fields of surgery, rheumatology, dermatology, endocrine, radiology, pathology, and nutrition
2.Co-management: IBD often overlaps with other inflammatory conditions and the disease states are often co-managed pharmacologically.
Minimum Qualifications
Qualification Requirements: For Assignment to Level I
1. Possession of a valid NY State license to practice pharmacy.
