Harlem Hospital Center has been a bedrock of the Harlem community since 1887. The hospital provides a wide range of medical, surgical, diagnostic, therapeutic and family support services to the residents of Central Harlem, West Harlem, Washington Heights and Inwood. Duties & Responsibilities

Under administrative direction, with the widest latitude for the exercise of independent initiative and judgment, supervise a complex Pharmacy unit or multiple Pharmacy units and/or performs as a specialty clinical pharmacist in a highly specialized field within a hospital, clinic or other health care setting. In addition to performing the duties of Assignment Levels I and II at a more difficult and responsible level, the following are typical tasks performed at Assignment Level III:

SUMMARY OF DUTIES AND RESPONSIBILITIES:
  • Provides supervision and technical/clinical direction to pharmacist and non-pharmacist employees in the Inpatient Pharmacy setting.
  • Monitors and evaluates staff performance and follows appropriate disciplinary proceedings as necessary.
  • Ensures staff compliance with regulatory agency and hospital rules and regulations.
  • Fills prescriptions and patient orders, resolves discrepancies in patient therapy, and follow-up on problems.
  • Utilizes clinical data in evaluating physician orders to ensure appropriate therapy provides and documents drug information and patient counseling.
  • Documents interventions, adverse drug reactions (ADRs) and medication errors as required by the institution.
  • Advises prescribers on alternate for non-formulary drugs.
  • Utilizes Outpatient and Inpatient computerized systems to enter and access patient care information.
  • Provides staff counseling for all technicians & Level 1, Level II pharmacists in regards to time and leave issues, non-compliance to sterility and other pharmacy practices & regulations as stated in Hospital and Pharmacy policies and procedures.
  • Ensures compliance with State & Federal laws, regulations, and standards regarding the provision of pharmaceutical services, and Pharmacy objectives, policies and procedures.
  • Re-enforces organizational and departmental policies and procedures on a day-to-day basis.
  • Conducts monthly staff meetings (as needed)
  • Oversees environmental and regulatory requirements of the Department and collaborates with housekeeping, Facilities, Infection Control, AOD to maintain proper operation.
  • Maintains daily workflow in order to expedite processing while ensuring quality, accuracy, and customer service
  • Troubleshoots and investigates intradepartmental and interdepartmental issues, complaints and concerns from healthcare providers, staff and patients; escalates issues as needed (to AOD, HP and Pharmacy Administration)
  • Demonstrates knowledge/skill in the following pharmacy areas, Main, Satellite, WAC, Code Cart, OPD and working knowledge of Narcotic/PYXIS and will cover these areas when staff is needed.
  • Ensures that the productivity and actions of that group meet/support the overall operational goals of the department as established by department leadership.
  • Responsible for assigned pharmacy services /activities including day-to-day operations, drug distribution, drug education, pharmacists' clinical activities, performance measurement/improvement, regulatory compliance, employee development and multi-disciplinary team development.

Minimum Qualifications

For Level I:
1. Possession of a valid NY State license to practice pharmacy.

For Level II: In addition to meeting the qualification requirements of the lower level:
1. Two years of post-licensure experience; or
2. Completion of a post graduate specialty residency or fellowship program; or
3. Current Pharmacy Board Certification.

For Level III: In addition to meeting the qualification requirements of the lower levels, an additional:
1. One year of post licensure supervisory experience or specialty clinical experience.
