Required Qualifications:

* PharmD from an accredited college or school of pharmacy.
* Coursework/Training Completion of a PGY-1 residency.

* 3 years experience in primary care pharmacy (or related area) involving direct, collaborative patient care.


* Upon hire: Oregon Pharmacist License
* Within 2 years of hire National Certification from Board of Pharmacy Specialties
* Upon hire: Washington Pharmacist License - based on clinic assignment
* Experience in writing, including research protocols, medical papers for publication, and educational pieces for patients and healthcare providers.
* Experience precepting students and/or residents.
