LPN Triage

Essential Functions:
* Obtains and records patient's vital signs and weight
* Assists Provider with patient examinations
* Performs medical procedures, and wound care, as needed.
* Administers and documents medication administration in patient chart (i.e., oral subconscious or intramuscular injection.
* Collects samples, such as urine, blood, and sputum, from patients for testing and performs routine laboratory tests on samples
* Reviews Provider's orders, lab requests, close out tasks, and Provider, Nurse and LPN buckets
* Interviews patients to determine medical problem/condition and documents in chart for provider.
* Assists patients with scheduling follow-up appointments
* Complete and submit prior authorizations for imaging, medications, Nexplanon, Paragard and other procedures.
* Processes patient's referral requests and conducts insurance verification (Pre-authorizations)
* Follow up on patient request as ordered by provider example medication refills telephone inquiries from affiliated agencies and other specialty providers as needed.
* Maintains health department STD treatment and verification records. Report other reportable diseases under the Philadelphia Health Department.
* Communicate with affiliated agencies regarding test results and updates on patient's charts, STI (treatment lists and verification of treatment).
* Assists with stocking and ordering new supplies and disposing of expired or damaged supplies
* Assists in administering COVID-19 testing, vaccines and education
* Sterilizes equipment and supplies
* Performs other duties as assigned
Required Skills/Education:
* One- year certificate from college or technical school for Licensed Graduate Practical Nursing; One year of practical experience in a community health center or an allied health field environment.
* Current state license.
* Active CPR Certification.
* Strong customer service skills.
* Effective verbal communication.
* Ability to time manage tasks and follow through with assignments.
* Display good judgment and confidentiality.
* Ability to problem solve.
