St. Luke's is seeking an Ambulatory Care Pharmacist to join our team for our downtown Boise Family Medicine Clinic.
What you can expect
* Workunder a broad collaborative practice agreement to provide chronic disease state medication management
* Manage a fast-paced clinic schedule of 8-12 patient appointments per day
* Provide support to providers and patients through answering drug information questions, trouble-shooting pharmacy problems and educating on medications
* Coordinate with a multidisciplinary team of physicians, advanced practice practitioners, behavioral health specialists and care managers
* Collaborate with clinical pharmacy technicians
* Opportunities for precepting pharmacy students and residents
Qualifications
* Degree from an accredited pharmacy program
* Valid Idaho Pharmacist License
Preferred Qualifications
* Previous experience with the clinical management of primary care based chronic diseases
* Completion of PGY2 Ambulatory Care OR completion of a PGY1 pharmacy residency program with ambulatory care-focus OR equivalent ambulatory care experience
* Current Board Certification in Pharmacotherapy (BCPS), Ambulatory Care Pharmacy (BCACP), or Geriatric Pharmacy (BCGP)
