Job Summary

We are seeking a Pharmacist to join our Ambulatory Pharmacy team. This pharmacist will be part of a newly created, multi-disciplinary team supporting our Primary Care South Nampa Clinic virtually from a clinic in Boise. In this position, you will be an essential team member of an outpatient healthcare team improving quality and patient outcomes by focusing on medication management.

Key Responsibilities
  • Serve as a valued member of our integrated, accessible healthcare services team, addressing medication needs and developing sustained partnerships with patients.
  • Tap into consumer insights to shape the care environment.
  • Coordinate with a multidisciplinary team of physicians, advanced practice practitioners, behavioral health specialists, and care managers, using a flexible mindset to adapt to consumer needs and preferences.
  • Serve pediatric, adult, and senior patient populations, addressing new health problems, chronic diseases, preventative care, and acute care not requiring an emergency room visit.
  • Coordinate with the pharmacy team to capture 340b medication referrals through pharmacotherapy/MTM services.
Qualifications
  • Degree from an accredited pharmacy program.
  • Valid Idaho Pharmacist License.
Preferred Qualifications
  • Clinical management of primary care-based chronic diseases.
  • Completion of PGY2 Ambulatory Care Residency or completion of a PGY1 pharmacy residency program with an ambulatory focus or equivalent ambulatory care experience.
  • Current Board Certification in Pharmacotherapy (BCPS), Ambulatory Care Pharmacy (BCACP), or Geriatric Pharmacy (BCGP).
