**Description**

The Community Health Worker III (CHW-III) is responsible for helping community residents and Medical Center patients and their families to navigate and access community services, other resources, and adopt healthy behaviors.

**Required Qualifications:**

+ Education to meet certification, license or registration requirement.

+ Driving may be necessary as part of this role. Caregivers are required to comply with all state laws and requirements for driving. Caregivers will be expected to provide proof of driver license and auto insurance upon request.

+ 2 years - Experience in a combination Community Health Worker I and II position, or 2 years related community outreach, enrollment, education or other community health administrative experience, or Bachelors degree in lieu of experience.

Providence Clinical Network (PCN) is a service line within Providence serving patients across seven states with quality, compassionate, coordinated care.
