The McGuire Group has an exciting opportunity for a Full-Time Admissions Screener to join our caring and knowledgeable team supporting our facilities in western New York. The admissions screener is works as a liaison between referral sources; and facilities, making both medical and social assessments, coordinating all admissions and transfers to the facility, working closely with the facility admission coordinator and a team of insurance coordinators and leadership in our admission departments.

The McGuire Group is comprised of 5 skilled nursing and subacute rehab facilities located throughout Western New York which includes five skilled nursing facilities We offer a variety of services including subacute rehab, long-term care, and hospice. We are proud to be part of Living Legends Health including facilities in Absolut Care, Vestra Care and Taconic Health Care.

ABOUT THE POSITION:

The successful candidate will perform a variety of duties, including but not limited to:

Responsibilities

* Responsible for completing admission assessments and recommending patients for long term or subacute care
* Visit designated hospital assignments daily
* Knowledge of age specific developmental factors specific to adult and geriatric residents (i.e. physical, cognitive, and socialization factors) in carrying out the admissions process
* Interacts with residents and family members, co-workers, clinical and ancillary staff in a non-judgmental, supportive and calm manner
* Additional tasks as assigned

Qualifications

* Degree in Nursing, Social Work or Physical Therapy preferred
* Experience in Admissions Screening and long-term care is preferred
* Good Understanding of Medicare and other Managed Care insurance helpful
* Must be flexible, professional, and reliable
* Computer skills including keyboarding and PCC
* Strong customer service skills
* Must possess a valid New York State Driver's License and have transportation available for daily job-related travel

EMPLOYMENT TYPE:

FULL-TIME
