Tuesday Health is looking for a creative and experienced leader to join our team as a Palliative Care Registered Nurse. You will work virtually alongside other members of our clinical team, providing a multi-disciplinary approach to care for our members with serious illness and ensuring the delivery of high-quality supportive care services to our patients and their families.

As a Registered Nurse, you will:

  • Virtually participate in visits for High/Medium acuity members
  • Be available to review care plans for low complexity patients with the patient's complex care navigator
  • Develop care plans for members enrolled in Tuesday Health services
  • Review documented observations and work with field-based nurses to establish a plan of nursing care
  • Oversee the implementation of the plan of nursing care
  • Support and coach field-based nurses on identifying interventions and support for members
  • Coordinate care with Tuesday Health nurse practitioner and community-based providers
  • Continuously monitor all signs, symptoms, and clinical data reported by engaged members via MyTuesday, the Tuesday Health symptom monitoring application
  • Triage concerns identified via MyTuesday including conducting video visits to assess urgent issues
  • Discuss urgent issues with the Tuesday Health nurse practitioner when clinical escalation is indicated
  • Interpret results and data and respond to members and care team regarding clinical interventions needed for each member
  • Support internal and joint rounds to ensure the member experience is optimal and coordinated
  • Learn the Tuesday Health electronic systems, tools, technology, to deliver a coordinated approach to ensure the members have a unique experience
  • Build a strong, trusting relationship with every member, where listening and empathy are the foundation of every interaction
  • Occasionally conduct in person visits with the most complex members

To be successful in this role, you will need:

  • Active and unrestricted Registered Nurse license in the State of Ohio without any board action
  • Palliative and/or hospice experience preferred
  • Experience in clinical/medical setting preferred
  • Experience in a multi- disciplinary care model working across network providers, health plan care management support, and employed clinicians
  • Strong communication and organization skills
  • Ability to multitask and work under pressure without sacrificing a positive member experience
  • Ability to know when to escalate and potentially act professionally in an emergent situation
  • Strong technical skills to navigate different documentation systems and tools; as well as ability to learn new systems as the company grows and evolves
  • Strong written skills
  • Comfort with change and ambiguity; you understand that rapid changes to the business, strategy, organization, and priorities will come with rapid evolution of our business

Schedule:

Full-time position with 40 hours/week

Monday - Friday with hours of 8:00am-5:00pm
