Location: Thome PACE

Overview

The LPN, Home Care Coordinator, has the responsibility of providing direct and indirect nursing care to PACE participants.

What you’ll do

Assessment:

Collect patient data and complete the required forms according to PACE standards.
Identify and prioritize participant problems and needs.
Conduct in-home assessments of prospective participants as assigned.

Planning Care:

Complete assessments of participants prior to scheduled team meetings.
Develop and revise care plans based on input from the IDT and participant/family involvement.
Make referrals to interdisciplinary support services as needed.

Implementing / Managing Care:

Assess home care and clinical needs, providing treatments as prescribed.
Conduct home visits for nursing care and manage medication needs.
Ensure adherence to care plans through regular in-home supervision visits.
Document services in the Electronic Medical Record and communicate participant changes to the team.

Leadership:

Attend clinic meetings and delegate nursing care as appropriate.
Participate in Quality Improvement (QI) processes and comply with safe working conditions.

What it takes

Required Qualifications
Graduate of an accredited School of Nursing.
LPN License
Must be CPR certified
