Absolute Hospice is looking for a Full-Time RN Case Manager for its team. This position is a great opportunity for the right candidate If you are a motivated, focused, RN looking for an opportunity to make an impact, this is an opportunity for you.
Responsibilities:
* Triaging calls
* Ensuring medical orders are transcribed and processed accurately
* Competence in evaluation of patient needs
* Promoting continuous quality improvement
* Teaching and counseling patients/families
* Timely and accurate documentation
* Case Managing
* Other duties as required
Requirements
* Graduate of an approved nursing program/school is required. Bachelor's Degree, preferred.
* At least 1 year of recent acute care experience in an institutional setting is required. At least 1 year of recent experience in Home Care is required.
* At least 1 year of Hospice experience is preferred.
* Active Michigan RN License is required.
* Active AHA BLS Certification is required.
* Responsible for supervising Hospice aides, supervisory/management experience is preferred but not required.
* Excellent observation, verbal and written communication skills, problem solving skills, basic math skills.
* Must possess a valid driver's license and acceptable driving record
