We have been asked to assist one of our top home care & hospice agency partners with a need for a Hospice Nurse Liaison to be onsite at a major Boston teaching hospital.

Full-time, 36 hours/week
3 12-hour shifts (9:00 a.m. - 9:30 p.m.)
Flexible schedule
Occasional weekend may be required

Responsibilities include:

Assessment of in-patient and determination of eligibility for Hospice
Admission for Hospice and daily patient visits
Creating and implementing individualized care plans
Coordinating and managing care with hospital nursing staff
Serving as Hospice Liaison between staff, case management, patients and families
Participating in weekly interdisciplinary team meetings
Educating patients and families about hospice

Position requires:

Minimum 2 years Hospice RN in a Medicare Certified Agency, Inpatient Hospice RN experience a plus.
MA RN License
RI RN License (or willingness to apply for it upon hire)

Must be comfortable with issues relative to end-of life care, death and dying.
