Paradigm Rehabilitation is seeking a highly enthusiastic and team-oriented Physical Therapist. Our primary goal is to make a difference in the lives of our patients. This Physical Therapy position is in the post-acute/SNF setting.

Your Responsibilities:
  • Screen & evaluate patients; Work to develop initial and ongoing treatment plan of care
  • Work with patients to optimize function while being willing to learn new ideas & approaches to care
  • Thorough documentation; timeliness and accuracy is key
  • Educate patients & staff; supervise and guide PTAs
  • Collaborate as an integral part of the care team
  • Understand how we partner with facilities, nursing, and other staff for the benefit of the patients

The Requirements:
  • Maintain a valid state license. Maintain awareness of issues related to the profession of Physical Therapy
  • Proficient in clinical documentation using Net Health/Optima or similar software
  • All healthcare workers must be vaccinated against COVID-19 or have an approved exemption/submit to regular testing per the CDPH order

Education:
  • Graduate of an CAPTE accredited school of Physical Therapy

Your Certificates/Licenses:
  • Valid Physical Therapist CA state license
