Job Summary

We are seeking a highly skilled and motivated Physical Therapist to join our team. As a Physical Therapist at Paradigm Rehabilitation, you will be responsible for evaluating and treating patients in a post-acute setting. Your primary goal will be to optimize patient function and improve their quality of life.

Key Responsibilities
  • Screen and Evaluate Patients: Conduct thorough assessments to identify patient needs and develop individualized treatment plans.
  • Develop and Implement Treatment Plans: Work with patients to set realistic goals and create effective treatment plans to achieve optimal outcomes.
  • Provide Patient Education: Educate patients on proper techniques and exercises to promote independence and self-care.
  • Collaborate with Healthcare Team: Work closely with nurses, physicians, and other healthcare professionals to ensure comprehensive patient care.
  • Document Patient Progress: Maintain accurate and timely records of patient progress and treatment outcomes.
Requirements
  • Valid State License: Maintain a current and valid Physical Therapist license in the state of California.
  • Clinical Documentation Skills: Proficient in using electronic medical records and clinical documentation software.
  • COVID-19 Vaccination: Comply with company policies regarding COVID-19 vaccination or submit to regular testing.
Education and Certifications
  • Graduate of CAPTE Accredited Program: Hold a degree from a Commission on Accreditation in Physical Therapy Education (CAPTE) accredited program in Physical Therapy.
  • Valid Physical Therapist License: Possess a valid Physical Therapist license in the state of California.
